How NDIS hospital discharge works
The NDIS sets out the steps in its guide to your hospital stay.
Tell hospital staff you are an NDIS participant
With your consent, they let the NDIS know you are there. If you are not a participant but may be eligible, they can help you apply.
The NDIS gets in touch
The NDIS says it will contact you or your nominee within 4 days of being told a plan change may be needed or that you want to apply. A specialised team, which may include a hospital liaison officer, works with you.
Your plan is updated
A specialised NDIA planner meets you to create a first plan or change your current one. The NDIS then tells the hospital which supports are in your plan.
The hospital makes the discharge plan
Hospital staff write the discharge plan and include your support coordinator. Your NDIS plan helps shape it.
You leave and your supports start
This is where a provider like us comes in: somewhere suitable to go and support workers ready on the day.
What the NDIS pays for in hospital
It is a common surprise, so it helps to know before you are there.
- Hospital staff
- Provide everyday care such as personal care and medicine. You are entitled to the same health care as everyone else.
- Your support worker
- NDIS funding generally cannot pay your usual support worker for care in hospital.
- The exceptions
- The NDIS says it can pay for supports in hospital only for a complex communication need or for managing behaviours of concern.
Where you can go from hospital
The right option depends on your plan, your needs and what is available.
Back home, with support
Support workers, home readiness and transport coordination so you can return to your own home. See our NDIS core support services in Melbourne.
Short-term accommodation
Somewhere suitable to stay while the home, equipment or longer-term arrangements are sorted. See NDIS short-term accommodation in Melbourne.
Supported independent living
Support workers in a shared home. See supported independent living in Melbourne.
Specialist disability accommodation
A purpose-built or modified home for eligible participants. See SDA in Melbourne and SDA housing across Victoria, and check current vacancies.
None of these pays your rent. SIL funds support workers, SDA funds the building for eligible participants, and rent and living costs are yours.
How we help
We are a registered NDIS provider in Melbourne.
- Discharge planning. We work with the hospital team, your support coordinator and your family on a plan for the day you leave.
- Home readiness. We check the home is ready, from access to equipment.
- Transport. We coordinate accessible transport from the ward.
- Support on the day. Support workers ready for your first days home.
- Somewhere to go. We help find suitable accommodation if home is not the right place yet.
- NDIS paperwork. We help you gather what the NDIA asks for.
- Follow-up. We check in after discharge and coordinate with your NDIS allied health team.
What we are not
We are not a hospital, nursing or rehabilitation service. We work alongside the hospital's clinical team so the move home is safe and supported.
Our discharge flyer
Share it with the family or the discharge team.
Everything we can do to get you from hospital to home, on one page. Print it or send it on to your support coordinator or the hospital social worker.

Talk to us early. A call before the discharge date gives everyone more time.
A short overview is enough to start. It helps if you can tell us:
- The hospital and ward, and the expected discharge date
- The person's support needs, including overnight needs
- Their NDIS plan status and who their support coordinator is
- Anything that has changed, such as behaviours of concern or new equipment
With consent we may also ask for the discharge summary, allied health reports, a behaviour support plan and medication information.

Questions people ask
What is NDIS hospital discharge support?
Help for NDIS participants to leave hospital safely, with the right home, support workers and plan in place. It covers planning with the hospital, somewhere suitable to go, and supports ready on the day you leave.
How quickly does the NDIS respond when someone is in hospital?
The NDIS says it will contact you or your nominee within 4 days after it is told that a plan change may be needed, or that you want to apply. You then work with a specialised hospital team, which may include a hospital liaison officer. See the NDIS guide to your hospital stay.
What is a hospital liaison officer?
A hospital liaison officer (HLO) is an NDIA staff member who works with hospital staff and you on your NDIS plan while you are in hospital. Read the NDIS page on what a hospital liaison officer is.
Does the NDIS pay for my support worker while I am in hospital?
Generally no. Hospital staff provide everyday care such as personal care and medicine. The NDIS says funding can pay for supports in hospital only for a complex communication need or for managing behaviours of concern.
Who can make a referral?
Participants, families and representatives, support coordinators, hospital social workers, discharge planners and allied health professionals. Call 1800 844 995 or use our referrals page. If you are unsure whether we are the right fit, call us and talk it through.
Can I go from hospital straight into SIL or SDA?
Sometimes, depending on your NDIS plan funding, eligibility and what is available. SIL funds support workers and only registered providers can deliver it. SDA funds the building for eligible participants. See supported independent living in Melbourne and SDA in Melbourne, or check current vacancies.
What if my home is not ready when I am?
Short-term accommodation can give you somewhere suitable to go while the home, equipment or longer-term arrangements are sorted. See our NDIS short-term accommodation in Melbourne.
Does the NDIS pay my rent?
No. Rent and everyday living costs are not NDIS supports. SIL funds the support workers in the home, and SDA funds the building for eligible participants.
Are you a hospital or medical service?
No. We are not a hospital, nursing or rehabilitation service. We work alongside the hospital's clinical team and your allied health professionals so the move home is safe and supported.
What should hospital staff send you?
A short overview helps: the hospital and ward, the expected discharge date, the person's support needs including overnight needs, their NDIS plan status and who their support coordinator is. With consent we may also ask for the discharge summary, allied health reports, a behaviour support plan and medication information.
Do you take urgent discharges?
Call us as early as you can and tell us the expected discharge date. We will tell you what is possible. Planning early gives the NDIA, the hospital and us the best chance to have everything ready.
